Question: Teenage Acne

My 17yr old daughter has (over the last year) suffered with quite bad acne around her mouth and on her forehead. I would like to know if there is procedure/cream that can be recommend - I heard that maybe a facial, peel may help

Answer: Hi Caroline, I'm sorry to hear your daughter has been suffering with bad acne. At the moment, creams would be the best way forward. The best way to get a grip with the problem is to start making the skin more acidic. As she is 17, we can't do any procedures in clinic, however, she can start using topical products to help her immediately. The best thing to do is to get an acidic skin wash, and an acidic moisturiser, along with an SPF. The Medik8 range is one we recommend.

The specific products are; Medik8 clarifying foam cleanser, and the refining moisturiser. Plus Heliocare gel spf 50

Use the products every other night at the start, then gradually build up to once in the morning and once at night. The spf only needs to be used in the mornings

Answer: Dear Caroline, there is a lot that can be done for acne. However it is very difficult to.civer it over a message. We will have to look at diet, lifestyle, skincare,etc. The good news is there is definitely things that can be done to help at home and in clinic. My recommendation would be to book a consultation with a Aesthetic Doctor to discuss your daughters case in detail.

Thank you for your message. Can I ask when your daughter will turn 18?
For insurance purposes, we can not perform our treatments on someone under 18. We can however see her for a consultation before her 18th birthday to discuss options that will soon become available to her.

Answer: Hello Caroline,

This is a common problem with teenagers, as hormonal changes are known to lead to skin disruption with acne.

It's important to recognise how acne forms on the skin, to understand how to treat it. Changes in hormone levels result in the skin follicles creating more sebum (oil), which mixes with dead skin cells and forms a mixture which clogs the follicle. Acne bacteria feed on this mixture and proliferate, and the body's immune system responds to this by increasing inflammation as a defence mechanism. Imbalance in gut bacteria can also result in proliferation of acne bacteria on the surface of the skin. Acne breakouts are formed as a result, with pustules and redness.

Consider topical treatments such as skincare, as well as oral probiotic supplements, and treatments such as chemical peels and LED therapy. Salicylic acid and lactic acid can help to exfoliate the congestion deep within the follicle, and the dead skin cells on the surface of the skin. Mandelic acid can help to reduce oil production. Skin peels such as trichloroacetic acid can provide deeper exfoliation. LED therapy can reduce the proliferation of bacteria on the skin surface. Probiotics can rebalance gut and skin bacteria.

It would be advisable to book a consultation first, to determine the factors causing the acne and to devise a personalised treatment plan.

Thank you for your question about treatments for acne. I'm really sorry to hear that your daughter has been suffering this last year. I hope the following information helps.

Acne can be caused by many factors, including bacteria, inflammation, dietary issues, hormonal imbalances, increased sebum output, and hyperkeratinisation (skin cells shed too much and block pores). Clinic treatments can be very helpful, but they are usually only part of the solution. Dorset Aesthetics offers patients holistic treatment advice, which takes into consideration lifestyle, diet and gut health, skincare products and routine, home treatments and mental well-being to offer a multi-faceted approach to treatment planning to give the best results.

In terms of treatments, I find regular superficial peels with hydroxy-acids work well in treating and preventing acne. They offer a chemical exfoliation together with a deep clean, but are well tolerated and don't cause visible shedding, so there's no downtime. Regular exfoliation is very helpful for acne, as it helps to reduce the build up of dead skin cells that block pores and cause spots. Chemical exfoliation with hydroxy-acids is preferable over a grainy, physical exfoliation, as harsh exfoliation can cause damage to the skin allowing bacteria to penetrate through the skin's barrier.

Keeping the skin hydrated with the right moisturiser is important, because if the skin is dry then it will produce more oil, and the oil won't be able to escape through thickened, dry skin caused by harsh exfoliation and drying products.

LED Phototherapy is a good option for acne, as it can reduce the amount of acne causing bacteria on the skin, reduce inflammation and redness, and stimulate healing. We use Dermalux in the clinic and offer the Deesse Pro Express LED mask for home use. These treatments are not suitable for patients on some antibiotics or accutane as they cause the skin to be very sensitive to light.

Firstly and most importantly I would like to advise towards booking in for a video or in clinic consultation as the best first point of process to asses your daughters skin professionally, to provide a more detailed and bespoke advice towards her current concerns to help guide towards her skin health.

However, due to your daughters age there is not a treatment we can provide or advise till she reaches the age of 18 years. But we can look at high strength home care options whilst providing a relationship of support for her with a skin specialist upon the cross assessment with one of our Doctors, which is often highly effective towards gaining the needed results.
The right everyday homecare skin care advice is vital to achieve the right results to gain Skin Health at any age. For example: products containing AHAs, BHAs, Retinols or chemicals SPFs which can be brought online without correctly trained guidance can likely cause further issues and not build the skin back in a healthy way, even if acne has seemed to been helped.
