As discussed, we have come across your clinic during an audit and noticed that you haven't been paying for the past year, I think there must have been some confusion as I spoke to you last year to say that we are swapping from standing order payments to direct debit monthly payments, however, it looks as though you cancelled down your standing order but didn't get around to setting up your direct debit payments.
