Launched in the U.K. in 2003, Bio-Alcamid™ is a medical device designed for the correction of small and large soft tissue deficits. Bio-Alcamid™ is used in reconstructive and cosmetic surgery both privately and within the NHS. Bio-Alcamid™ is marketed as a soft tissue "endoprosthesis". Once implanted, Bio-Alcamid, unlike many other injectable fillers, becomes covered by a very thin collagen capsule (0.02 mm) which completely surrounds the gel, isolating it from the host tissues and making it a real endogenous prosthesis.
Bio-Alcamid™ is a gel polymer comprising exclusively of networks of alkyl-imide groups (approximately 4%) and nonpyrogenic water (approximately 96%). Alkyl-imide belongs to the family of acryl-derivatives and its polymeric structure does not contain free monomers.

It is a permanent filler where the volume of the implant is given by water not from a solid product. As the initial swelling subsides there may be some loss of volume, but with the appropriate injection technique one visit would normally only be required to achieve the desired aesthetic result.

Bio-Alcamid™ is indicated in all cases in which a correction of a soft tissue deficit or reconstruction of an anatomical structure affected by congenital or acquired loss of soft tissue is required.
Unlike many other injectable fillers currently available, Bio-Alcamid™ can be used to correct small or large deficits such as Pectus Excavatum, Poland`s Syndrome and liposuction deficits The LIP form is soft and compact, and its viscosity is ideal for correcting the lip outline to make the injected implant imperceptible to the touch.
Bio-Alcamid™ in its LIP form can only be used to correct and project the labial profile because Bio-Alcamid™ must not be injected into the mucosa. The FACE form has the same composition (it is not a dilution), but the viscosity is suitable for correcting medium and medium-small volume deficits such as zygomatic and genial hypovolumetry, prominent naso-labial fissures, etc. The BODY form has the same consistency as the FACE form, but contains more material to satisfy the need to correct significant tissue deficit such as facial atrophy, Poland syndrome, Pectus excavatum, gluteal atrophy, hypovolumetry of the calves, etc. One of the advantages of this endoprosthestic material is that large quantities can be injected to suit the different needs without any risk of the common undesirable effects created by other similar materials.
Pregnancy or lactation and patients with pre-existing infections.
The effect of the injection of Bio-Alcamid™ is permanent. As the initial swelling subsides there may be some loss of volume, i.e. there will be some return of the wrinkle, crease, scar or lip shape. For this reason occasionally an additional treatment may be necessary to obtain an optimum result. Since the ageing of the skin continues, it may also be necessary to carry out follow-up injections after a certain period of time, depending on a number of factors (skin structure, age, etc...). Bio-Alcamid™ can be removed shortly after the injection if necessary. If the gel has to be removed at a later stage, the manufacturer claims that this procedure is relatively simple and safe.
The swelling that develops immediately after the implant is minimal under normal conditions. Immediately following the injection, there is a slight reddening in the area, but this disappears after a few minutes.
There is additional swelling a few hours after the procedure is completed. The positive (acute) self-controlled inflammatory response begins within a few hours and ends approximately 8 weeks later when the encapsulation process is complete. The resulting appearance of the implanted areas improves with each passing day. Upon the encapsulation being complete the overall effect is natural both in appearance and to the touch. Bio-Alcamid™ has not, to date, been associated with any significant complications according to Polymekon the manufacturer; however we are aware of anecdotal reports from people who have received the product which include granulomas and product migration which the manufacturer argues is due to the incorrect placement of the product by practitioners.
The manufacturer claims that the exceptional biocompatability of the product excludes the possible development of granulomas. Infection, however, is a potential risk associated with the implantation of any prosthesis.

Research for new biocompatible and easily implantable materials continously proposes new molecules and new substances with biological, chemical and physical characteristics more and more adapted to aesthetic and reconstructive surgery. Bio-Alcamid™ (Polymekon, Italy) is a non reabsorbable polymeric material composed of alkylimide-amide groups. This cosmetic agent has been used for the treatment of 2000 patients by a multicentric trial led by different Hospitals and University in Italy and abroad. Very serious aesthetic defects such as pectus excavatum, Poland Syndrome, postoperative traumas, in addition to common aesthetic defects such as lips, cheek-bone and chin hypovolumetry and relaxing of nasolabial sulcus, have been treated by surgical implant of Bio-Alcamid™. Aesthetic results were excellent; tissues felt soft and the implants were uniformly distributed. No migration or dislocation of the implants, no granulomas, no allergic response and no kind of intolerance were observed. Only 12/2000 patients had post-operative complications (Staphylococcus infections) and only 3/12 cases could be directly ascribed to the implanted material. For its characteristics Bio-Alcamid™ can be considered a novelty in the aesthetic and reconstructive surgery; it is absolutely biocompatible, non toxic, non allergenic, easily injectable and quickly removable. Bio-Alcamid™ can be defined as a sort of "endoprosthesis", perfectly suitable for soft tissue augmentation and for the correction of different tissue deficiences, with a long-term safety and efficiency.
